    In accordance with Public Act 98-1111, Section 15.1 of the Illinois Optometric Practice Act of 1987 (225 ILCS 80/15.1) has been amended to permit an Illinois Licensed Optometrist to prescribe Dihydrocodeinone (Hydrocodone) with one or more active, non-narcotic ingredients, only in a quantity sufficient to provide treatment for up to 72 hours. All other uses of Schedule II controlled substances are prohibited.
    An optometrist wishing to add Schedule II to an existing Illinois Optometrist Controlled Substances license must return the ORIGINAL Illinois Controlled Substances License with a signed letter of request that includes his/her name and license number or SSN. A photocopy may be posted until the new license is issued; the ORIGINAL must be returned. No fee will be required for this change. Requests may be mailed to:
